Workers’ Comp Centralizes Designated Doctor Reports, Narratives

As of March 1, 2011, Texas workers’ comp designated doctors must fax reports and associated narrative attachments to a single, central fax number: (512) 490-1047.

Designated doctors may begin using this fax number at any time. After March 1, do not   send these reports and narratives to Texas Department of Insurance, Division of Workers’ Compensation (TDI-DWC) field offices or other agency fax numbers.

Note these forms:

  • TDI-DWC has updated (PDF) Form-069, Report of Medical Evaluation, and DWC Form-073, Work Status Report. Physicians must begin using this form starting March 1.
  • Recently adopted rules require that designated doctors complete Form-073, Work Status Report, when evaluating an injured employee’s ability to return to work. Submit the forms to TDI-DWC as a part of the designated doctor report at the fax number listed above.
