Shingles

Physicians bill Medicare Part D for both the vaccine and the administration; coverage and copays vary by the patient’s Medicare Part D plan. Use these codes:

  • CPT codes — vaccine: 90736, Zoster (shingles) vaccine, live, for subcutaneous use;  administration: 90471
  • ICD-9  code —  V04.89 or V05.8
