    Are you a practice owner concerned about the cost of health insurance? Complete the short form below to speak with a TMA Insurance Trust advisor about limited-time waivers and other options that could reduce the impact of employee health insurance on your practice's bottom line.

    • Employer Group Coverage. TMA Insurance Trust will research available PPO and HMO options and help you choose a plan. We will also enroll your staff and their dependents, distribute temporary ID cards and policy information, and assist throughout the year with billing, claims or customer service issues with the carriers. Select the "Medical Group" option on the form below.
    • Special PPO option for individuals who qualify as a "group of one." If you own a medical practice with a tax ID number and have at least one employee (other than yourself), we may be able to help you obtain a group PPO plan just for yourself and your family. If you meet these requirements, please talk to our advisors about the "group of one" option, or select it on the form below.
    • Partners and LLCs Filing as a Partnership with No Employees. Partners and LLCs filling as a partnership with no employees may also be eligible for group coverage for themselves and their families. To qualify, a physician needs to provide partnership documentation or the company's Schedule K-1 (Form 1065). If this describes your situation, please complete the form and select the "Partnership/LLC Owners" option on the form below.
    • Individual and Family HMO Plan. From November 1 through December 15, you can enroll in an individual HMO plan with an expanded network for yourself and your family.
