Texas workers’ compensation designated doctors must fax reports and associated narrative attachments to a single, central fax number: (512) 490-1047.
Do not send these reports and narratives to Texas Department of Insurance, Division of Workers’ Compensation (TDI-DWC) field offices or other agency fax numbers.
Note these DWC forms:
- TDI-DWC Form-069, Report of Medical Evaluation,
- TDI-DWC Form-068, Designated Doctor Examination Data Report, and
- TDI-DWC Form-073, Work Status Report, for use when evaluating an injured employee’s ability to return to work.
Submit the forms to TDI-DWC as a part of the designated doctor report at the fax number listed above.
