easy-to-use resources to help your patients achieve their New
Year's goals. The comprehensive toolkit includes patient
questionnaires, action plans, progress calendars, and more. It
focuses on these four key behaviors:

- Increasing physical activity,
- Quitting smoking, and
- Reducing risky drinking.
