How Would You Change the 1500 Form?

The National Uniform Claim Committee (NUCC) is asking physicians, insurers, and others in the health care industry to complete a survey on potential changes to the 1500 Health Insurance Claim Form. It says the changes are based are based on claim reporting changes in the 837 Professional Version 005010 transaction and other business needs.

Many of the proposals involve adding or deleting data elements. Another proposed change is increasing the number of diagnosis codes that can be reported from 4 to 12.

Nov. 20 is the deadline to complete the survey.

Chaired by the American Medical Association, NUCC is a voluntary organization representing physicians, providers, payers, designated standards maintenance organizations, public health organizations, and vendors. NUCC replaced the Uniform Claim Form Task Force in 1995. It develops a standardized data set to transmit claim and encounter information to and from all third-party payers.
