Contacting TrailBlazer? Have These Three ID Numbers at Hand

Starting April 6, 2009, you will need to provide three identification numbers when you call the TrailBlazer Interactive Voice Response (IVR) system or contact a TrailBlazer customer service representative (see phone numbers and hours of operation below):

  • Your National Provider Identifier (NPI),
  • Your Provider Transaction Access Number (PTAN), and
  • The last five digits of your tax identification number (TIN).

Note that callers will be allowed only three attempts to correctly provide these numbers.

PTAN Tips

The PTAN is your Medicare legacy number (sometimes call the Medicare PIN).  If you've forgotten you PTAN, you can look it up on a past Medicare remittance advice.

Some PTANs contain both letters and numbers. You must press the star (*) key to let the IVR know you are entering a letter. Press the key with the letter you wish to enter and then press the number 1, 2, or 3 depending on the position of the letter on that key. For example: A = *2 and 1.

Below is a "cheat sheet" list of letters. Or, you can use TrailBlazer's PTAN and name-to-number conversion tools to help with entering your PTAN as well as the patient's last name and into the IVR 9 (e.g., type the patient's name into the tool, and it will show you the equivalent number on a telephone keypad).
