The Centers for Medicare & Medicaid Services (CMS) issued revised CMS-855 Medicare enrollment applications [PDF] in March. Among the changes is removal of the requirement that physicians attach their national provider identifier notification received from the National Plan and Provider Enumeration System.
CMS says physicians should begin using the new application form immediately.
