Brush Up on Patient Records Requirements for Meaningful Use

The Centers for Medicare & Medicaid Services (CMS) requires 80 percent of a physician's patients to have records in the certified EHR technology (CEHRT) to meet certain objectives and measures of the electronic health record (EHR) incentive program. For example, the following Stage 1 measures require that more than 80 percent of patients seen by the physician have at least one entry in the EHR: 

  • Up-to-date problem list of current and active diagnoses;
  • Active medication list; and
  • Active medication allergy list.
